Here is the compatibility fact that flips everything you thought you knew: O is the universal red-cell donor, and AB is the universal plasma donor. Same blood, same person, opposite superpowers. Understanding the flip takes one idea: red cells are judged by their antigens; plasma is judged by its antibodies.
Red cells: judged by what they wear
A transfused red cell is attacked if it displays antigens the patient’s immune system doesn’t recognise. So the safest red cell is the bare one (O−) with no A, no B, no D. It fits into everyone.
An AB+ red cell is the opposite: wearing every common antigen, it fits only the one patient type that recognises all of them, other AB+ people.
Plasma: judged by what it carries
Plasma is the liquid, and its danger is the antibodies dissolved in it. Give group-O plasma to a group-A patient and the plasma’s anti-A antibodies start attacking the patient’s own red cells.
Group AB plasma contains neither anti-A nor anti-B, nothing to attack anyone. AB plasma suits every patient. It is precious for burns, liver failure and clotting emergencies, and blood banks never have as much as they want, because AB donors are a minority. Why banks prize AB donations →

- AB donors: universal plasma donors, and the first candidates for apheresis, where plasma or platelets are collected and red cells are returned to you.
- Everyone else: precisely matched red cells for the patients who share your antigens, which, in India, is a very large family.
One caution
