Red blood cells, the oxygen couriers

What red blood cells do, why their shape and haemoglobin matter, how long they live, and why they're the heart of every blood transfusion.

Every second of your life, a delivery fleet is working inside you. It has no days off, no sick leave, and it numbers in the trillions. Red blood cells (also called erythrocytes) are the tiny couriers responsible for one thing: carrying oxygen from your lungs to every tissue in your body, then hauling carbon dioxide back out. Without them, life as we know it simply wouldn’t run.

A shape built for the job

Look at a red blood cell under a microscope and you’ll see something unusual: not a sphere, but a biconcave disc, round, flattish, and dented in the middle on both sides, like a doughnut that never quite got its hole. That shape isn’t an accident.

A biconcave disc gives the cell more surface area relative to its volume than a ball would, so oxygen can move in and out faster. The thin middle also lets the cell flex and fold, squeezing through capillaries narrower than itself. Red cells are remarkably deformable, they bend, twist, and pop back into shape, millions of times over their lifetime.

Haemoglobin: the cargo hold

Inside each red cell, roughly 270 million molecules of haemoglobin are packed in. Haemoglobin is the iron-containing protein that actually grabs oxygen in the lungs and releases it where it’s needed. Here’s the clever part: red blood cells have no nucleus. As they mature in the bone marrow, they eject it. Losing the nucleus means losing the ability to divide or repair themselves (a fatal trade-off for most cells) but it frees up more room for haemoglobin. Each cell becomes a dedicated oxygen tank rather than a general-purpose cell.

A short life, endlessly renewed

A red blood cell lives about 120 days. Over those four months it makes an estimated 170,000 laps around your circulatory system, getting squeezed through capillaries and banged against vessel walls until it wears out. Old cells are filtered out by the spleen and liver, and their iron is recycled to build new ones.

What high or low counts mean

A routine blood test counts your red cells (along with haemoglobin and related measures). Results outside the usual range point broadly to what’s happening in your body:

  • Low counts often suggest anaemia (from iron deficiency, blood loss, or problems producing cells) leaving you tired, breathless, or weak.
  • High counts can occur with chronic low oxygen (such as smoking or living at altitude), dehydration, or bone marrow conditions.

None of these are diagnoses by themselves; doctors read the numbers alongside the rest of your health picture. But they’re often the first clue that the delivery fleet needs attention.

Why red cells are the main event in transfusion

When someone bleeds heavily in an accident, or their marrow fails, or their haemoglobin crashes before surgery, the single most useful thing another person can give them is red blood cells.
