Every blood group card in the world comes from the same tidy two-step lab trick. Forward grouping reads what’s on your red cells; reverse grouping reads what’s in your plasma; and the two must agree before a lab prints your group.
Step 1 — Forward grouping (your cells)
The lab mixes your red cells with two known antibodies: anti-A and anti-B.
- Cells clump with anti-A → you have the A antigen
- Cells clump with anti-B → you have the B antigen
- Clump with both → AB; with neither → O
Then anti-D is added: clumping means Rh-positive, no clumping means Rh-negative.
Step 2 — Reverse grouping (your plasma)
The mirror test. Your plasma is mixed with known A-cells and B-cells:
- Your plasma attacks A-cells → you carry anti-A → you are group B
- Your plasma attacks B-cells → anti-B → you are group A
- Attacks both → O; attacks neither → AB
Why the double test matters
Your body writes its group into your plasma as antibodies — you carry antibodies against the groups you don’t have. If forward says “A” but reverse says “B”, something is wrong and the lab repeats everything. This built-in cross-check is why a correctly processed grouping result is trusted for life.
