If you are DONATING blood — bring one thing
A photo ID showing you are 18+. Aadhaar, driving licence, voter ID, PAN, passport, college ID — any one. That is the complete list. No health certificate, no blood-group card, no doctor’s note, no parental letter. First-time or fiftieth-time, the requirement is identical.
If your PATIENT is receiving blood — bring three things
- The doctor’s blood requisition slip — the slip your treating doctor writes stating the component, group and units needed. The blood bank cannot issue without it.
- Patient identification — hospital admission card/ID.
- Attendant’s photo ID — whoever is carrying the requisition.
If available, also bring any previous blood-group report of the patient — it speeds up crossmatching, though the bank re-tests regardless.
What you do NOT need (kill these myths)
- No “donor card” needed to donate — you get one after donating
- No fitness certificate — screening at the centre covers it
- No relationship proof for family donation at most banks (replacement credit usually takes any group)
- No money — processing charges, where they exist, are billed to the hospital bill, never paid in cash at a counter to a person
That is everything. One ID to give blood; a slip, an ID and an attendant to receive it.
